Disclosure request details
Under Schedule 2, Part 1 Paragraph of the Data Protection Act organisations that have a crime prevention, law enforcement or tax collection function may require personal information held by the council to prevent or detect a crime, or apprehend or prosecute an offender, or for taxation/benefit purposes.

Any organisation that has a lawful basis and legitimate need can submit a request; this is usually organisations such as:
· Child Support Agency
· Department for Work and Pensions
· Health and Safety Executive
· HM Revenue and Customs
· Local Authorities or Public Bodies, acting under authorised powers
· Police Authorities; and
· Regulators, such as OfSTED.

Advisory Teaching Service
The Advisory Teaching Service works with schools to provide specialist support to enable children with additional needs to reach their full potential.
The ATS work in partnership with Teachers, Parents, Healthcare Professionals and other Educational Colleagues to ensure the best care and support is provided to every child with additional needs.
Parents and Carers are able to request support from the ATS on behalf of their child.

Safeguarding
May hold information in relation to investigations they have undertook to protect Children and vulnerable Adults.

Education Psychology
If a child needs an educational psychology assessment, schools and other educational settings are able to request support from the Gloucestershire County Council's Educational Psychology Service.

Legal
May hold a Legal Index if they have had involvement.

Education
CAPITA holds limited information relating to students school attendance and grades.

Childrens Social Care
Children's social workers will work to protect children and young people and enable them to develop resilience through responsive relationships. They work inclusively with children services colleagues and partners to improve outcomes for children and young people.

Adults Social Care
Making sure adults (aged 18 or over) and their carers (including young carers) who need care or support in their daily lives can get the help and advice they need to live as independently as possible. Care and support can be a mixture of practical, financial and emotional support for people who need extra help to manage their lives and be independent

Special Educational Needs (SEN)
The Special Educational Needs and Disability (SEND) team works in accordance with the SEND Code of Practice to provide statutory guidance for organisations who work with and support children and young people of ages 0-25 with special educational needs and disabilities. They specifically hold information and documentation related to the provision, of and implementation of, special education and disability support.

Youth services
YOT works with vulnerable young people in the county, targeting specific groups who are most at risk of not making a successful transition into adulthood. This includes young offenders and those on the edge of the criminal justice system, looked after children and care leavers over the age of 16, unemployed young people, homeless young people, teenage parents, young people who run away from home or care, young people with disabilities and/or learning difficulties and young people with substance misuse issues or other health needs.

You need to ensure that your request includes:
· A countersignature from a senior officer within your organisation.
· Requests for disclosure from the Police need to be countersigned by an officer one rank above the requesting officer or higher.
· For other organisation this needs to be a team manager or the equivalent.
· Details of any legislative requirement or provide the information.
· What legislation or legal gateway applies to the requested access; and
· An explanation as to why the information is necessary to meet that obligation or legal gateway.

Failure to complete the form fully is likely to delay the process of processing your request.

Information required because

Your request will be reviewed to ensure there is a lawful basis for us to process the request. Once this has been confirmed we will send an acknowledgement email as confirmation we will process your request; we will then proceed with retrieving any appropriate information held.

When the information has been collated you will be sent a link to the information in a read only format. This link will be open to you for two weeks. Once you have reviewed the information and informed us which pages you need, we will send you a copy via secure email.
