Personal information request
We need you to supply two forms of proof of your identity.

Please supply one of the following copy documents to confirm your identity:
  • a copy of your birth certificate; or
  • a copy of your passport; or
  • a copy of your driving licence; or
    Please also supply one of the following copy, dated within the last 3 months, to confirm your address:
  • a copy of a Utility bill
  • a copy of a Bank statement
  • proof of change of name (if applicable)

  • Once you have your proof of identity ready you can complete this form, please be aware we will be unable to process your request until we have received these.

    You don't have an automatic right to access the personal information of someone else, but there are circumstances when you can do so, such as where you have parental responsibility (and they are aged under 13) or where you have a legal power, such as through a lasting power of attorney.
    In addition to the requirements stated above regarding proof of your identity, you must also provide evidence of A. the Data Subject's identity and B. your right to make this request on their behalf. Without this we will be unable to start processing your request.

    Where the Data Subject is 13 years old, or over you must provide either written or legal authority to act on their behalf. Written signed consent from the data subject or legal authority, please describe the legal authority you have and include a certified copy along with this submission.

    Please note there are two types of Lasting Power of Attorney (LPA); Property and Financial Affairs and Health and Welfare. Please ensure you provide the correct one for each data type you are requesting, e.g., if you are requesting social care records and financial assessment records you will need to provide an LPA for both Health and Welfare and Property and Financial Affairs.

    Where the Data Subject is under the age of 13 years you will need to prove that you have parental responsibility for them. Full birth certificate (if you are providing their full birth certificate as proof of identity which shows your name as a parent this will be enough) OR Document that awards you parental responsibility, e.g. a Court Order. Please describe the type of document you have and include a certified copy along with this form.
    When making an application on behalf of a person who is unable to understand the Rights of Access, Rectification and Erasure process, you must do so only with their best interests in mind.
    We will consider whether the provision of information is in your or another person's best interests. This means that even if you can prove the right to request information, we may not be able to give it to you.

    If you are requesting the right to rectification in relation to a record of opinion, such as from a professional on a social care record, we will ensure the record shows clearly that the information is an opinion and, where appropriate, whose opinion it is. We will ensure that your opinion in is added to the record. This will ensure that both opinions are available for use in the future and for any further decision making.
